Application Form and Requirements

1. Personal Details

  • Full Name (as in your passport)
  • Date of Birth
  • Nationality
  • Address
  • Contact Information (email and phone number)

2. Passport Details

  • Copy of your valid passport
  • Entry visa (if applicable for the country you are applying from)

3. Letter of Request

  • A notarized letter addressed to the Ambassador, requesting the issuance of a consular card.

4. Additional Required Documents

  • Copy of national identity card or any government-issued ID
  • Two (02) 4×4 photographs with a white background
  • Proof of residency or legal stay in the country
  • Proof of profession
  • A legalized copy of the birth certificate
  • A legalized application addressed to Madam the Chargé d’Affaires a.i., including the full address and telephone contact details of the applicant(s)
  • A copy of a valid passport

5. Payment of Fees

  • Application Fee (if applicable) – Ensure you check the applicable fee for the consular card.

6. Proof of Purpose

  • If needed, provide a letter stating the purpose for the consular card (e.g., family or business-related).

Application Process

Step 1: Fill Out the Application Form

Step 2: Submit Required Documents
